Abstract
Introduction. Known methods of fixation involve attaching the membrane to the surrounding tissues using pins and suture material. Such methods are voluminous, difficult to perform and not ergonomic, which increases the time of the operation, provokes additional stress for the patient and fatigue of the operating surgeon. This is related to the technical difficulties that arise while modeling and fixation the barrier membranes around the teeth during the closure of wall and furcation defects, and the duration of the surgical intervention itself. The aim of the study — to evaluate the effectiveness of the developed method of fixation of barrier membranes, which is used in the treatment of patients with generalized periodontitis by the method of guided tissue regeneration. Materials and methods. In the course of the study, 60 patients with generalized periodontitis aged from 29 to 50 years old, 25 (41.6%) men and 35 (58.3%) women, were examined. The criteria for the inclusion of patients in the study were the presence of generalized periodontitis II-III degree of severity, for which surgical treatment by the guided tissue regeneration method is indicated in the complex treatment, individual consent of the patient. Various research methods were used to evaluate the effectiveness of treatment of patients with generalized periodontitis by the guided tissue regeneration method, in particular, measurement of periodontal pocket depth (PPD). Results and discussion. According to our algorithm, the treatment of patients with generalized periodontitis was complex and phased. We have improved the stages of the guided tissue regeneration method in the treatment of patients with generalized periodontitis. The improvement consists in the development of new ways of fixating barrier membranes, adapted to the height of the cellular process or the cellular part of the jaws. Conclusions. The proposed method of fixation of barrier membranes is appropriate in the treatment of patients with generalized periodontitis by the guided tissue regeneration method. A clinical trial of the fixation method established an increase in the effectiveness of the treatment of patients with generalized periodontitis by the guided tissue regeneration method. Positive results were achieved in clinical situations where traditional methods were ineffective. Improvement of methods of fixation of barrier material, their combination with modern osteoplastic materials, the latest methods of scaling and modification of exposed surfaces of tooth roots, the use of biologically active materials contributed to increasing the effectiveness of treatment of patients with generalized periodontitis by the guided tissue regeneration method.
